Measles, Mumps, Rubella, Varicella Vaccines

Patient Compensation: $50 for each visit
Length of Study: 8 months
Number of Office Visits: 3

Criteria:
1) Is your child between the ages of 12-15 months?
2) Has your child received a vaccination against measles, mumps, rubella, and/or varicella?
3) Does your child have a history of measles, mumps, rubella, and/or varicella/zoster disease?
4) Does your child have any known illnesses or allergies?
5) Has your child been vaccinated against Hepatitis A?
6) Is your child currently living with a pregnant woman with a negative history of
chickenpox disease and without recorded vaccination against chickenpox?
7) Is your child currently living with new-born infants (0-4 weeks of age)?

2) Have you recently been diagnosed with Diabetes II?
3) Are you taking any diabetes II meds?
4) Have you been diagnosed with Diabetes I?
5) Do you have any know illnesses?
6) Are you currently participating in a clinical trial?
7) Are you on a weight loss program or taking weight loss medication?
Are you currently pregnant or breastfeeding?
9) In the last 6 months have you had a history of drug or alcohol abuse?

Patient Compensation: $50 a visit
Length of Study: 14 weeks
Number of Office Visits: 7 visits

Criteria:
1) Are you between the ages of 18-80?
2) Have you been diagnosed with Diabetes II? Has your diagnoses occurred in the last 2
years?
3) Have you been diagnosed with Diabetes I?
4) Are you taking any medications for your Diabetes?
5) Are you currently taking insulin?
6) Do you have any know illnesses?
7) Are you currently participating in a clinical trial?
Are you currently pregnant or breastfeeding?
9) In the last 6 months have you had a history of drug or alcohol abuse?

2) Yes (If diagnosed in the last 2 years: can’t be under the age 25 or have a BMI less
than 25)
