Number of Office Visits: 9-10
Criteria:
1) Are you at least 40 years old?
2) Have you had a history of COPD for at least 1year?
3) Are you a current or past smoker?
4) Is your rescue medication Albuterol?
5) Females: Are you currently pregnant or breastfeeding?
6) In the last 6 months have you had a history of drug or alcohol abuse?

Number of Office Visits: 14

Criteria:
1) Are you at least 40 years old?
2) Have you had a history of moderate to severe COPD?
3) Do you have a history of smoking cigarettes for at least 20 years?
4) Do you use oxygen for over 15 hours a day?
5) Have you had a respiratory tract infection within the last 6 weeks?
6) Have you been hospitalized in the last 7 weeks due to your COPD?
7) Do you have a history of lung cancer?
Do you have Diabetes?
9) Females: Are you currently pregnant or breastfeeding?

10) In the last 6 months have you had a history of drug or alcohol abuse?
