1) Are you at least 18 years?
2) Are you currently sick with the flu?
3) Do you have a fever?
4) Do you have symptoms of the flu such as: coughing, score throat, headache, or
fatigue?
5) Do you suffer from COPD?
6) Have you received a flu vaccine within the last 21 days?
7) Are you currently pregnant or breastfeeding?
In the last 6 months have you had a history of drug or alcohol abuse?
