5) Have you ever undergone surgical or device intervention for your weight?
6) Do you smoke cigarettes?
7) In the last 3 months have you used nicotine products or weight loss products?
Are you currently pregnant or breastfeeding?
9) In the last 6 months have you had a history of drug or alcohol abuse?

3) Do you have any Psychiatric illnesses? Such as Schizophrenia, Depression, or Anxiety?
4) Do you have Diabetes?
5) In the last 3 months have you gained or lost 10 pounds?
6) Have you ever undergone surgical or device intervention for your obesity?
7) Have you smoked cigarettes in the last 3 months?
Are you currently pregnant or breastfeeding?
9) In the last 6 months have you had a history of drug or alcohol abuse?

Answers to qualify:

2) Must have:
BMI: 27-30 okay if have controlled blood pressure and/or 6 weeks stable on cholesterol
meds
