Criteria:
1) Is your child between the ages of 2-11 years old?
2) Has your child been diagnosed with Asthma?
3) Does your child have a history of life threatening asthma?
4) Is your child allergic to any Asthma medications?
Answers to qualify:
1) Yes
2) 6+ the child must have been diagnosed at least 2 years ago & 2-5 years old the child must have
been diagnosed at least one year ago
