2) Do you have at least one pressure ulcer on your body?
3) Do you have diabetes?
4) Are you currently undergoing immunotherapy or chemo therapy?
5) Are you currently pregnant or breastfeeding?
6) In the last 6 months have you had a history of drug or alcohol abuse?
