2) Have you been diagnosed with Diabetes II?
3) What is your height and weight?
4) Are you currently taking insulin?
5) Do you have a history of any psychiatric illnesses?
6) Have you been diagnosed with HIV, Hepatitis B or C?
7) Are you currently pregnant or breastfeeding?
In the last 6 months have you had a history of drug or alcohol abuse?
